This will require providing personal information, verifying their subscription and state of residence, and choosing an electronic payment method.
Individuals who did not receive a notice but believe they are eligible for a payout can contact the administrator via email at [email protected]. Alternatively, they can print a PDF version of the claim form and return it by mail to the settlement administrator's office.
The claim form must be completed online or postmarked by September 8, 2026, to be eligible for a payout.
